Our client is a reputable Non-profit Organisation that provides a wide variety of welfare and social services. They are currently expanding their professional consulting team and are now looking for a Care Coordinator (RN).

- Coordinate and provide nursing care including health assessment and screening, health coaching, planning and implementation of chronic diseases management and community rehabilitation programmes
- Develop clinical guidelines and workflow of nursing services
- Collaborate with multi-disciplinary team to develop and conduct health promotion and patient education programmes and related training
- Liaise with government officials, network medical practitioners, professional groups, district stakeholders and related parties
skills & experiences required.
Holder of valid Part I or II Registered Nurse practicing certificate issued by the Nursing Council of Hong KongDegree in Nursing or equivalentMinimum 5 years of aggregate experience as registered nurse with supervisory experience in patient care related nursing practice, preferably in public services / NGOsRecognized post registration training in Primary Health Care / Community Care / Diabetes Mellitus care / Cardiology / Stroke Rehabilitation / Cardiac Rehabilitation / Gerontology are advantagesCandidates with less experience will be considered as Registered NurseInterested individuals can click apply now and send through an updated resume in WORD format.
